Record correction
Definition
Fixing an error in documentation in a way that keeps the original visible, such as one line through the mistake on paper or an amendment in an electronic record, with the date and the writer's name or initials. Errors are never erased or hidden.
Example from a session
The RBT draws one line through "45 min," writes "30 min," and adds her initials and the date.
